Projet de réaffiliation en itinérance et santé mentale (PRISM)

What is the mission of the PRISM team?

Located at the Welcome Hall Mission in Montreal, PRISM is a mental health reinsertion project for adult men experiencing homelessness along with symptoms of schizophrenia, bipolar spectrum disorders, or other psychotic disorders. PRISM aims to provide users with transitional, adapted services in their own environment to help them begin their recovery process, reconnect to specialized mental health services, and transition to their own apartments.

The PRISM team

PRISM’s interdisciplinary team includes:

  • A psychiatrist
  • A psychoeducator specialized in drug addiction / mental health
  • A nurse
  • A designated Welcome Hall Mission worker

Connexion/Connexion mixte

The Connexion team

The interdisciplinary Connexion team reaches out to people experiencing homelessness or at high risk of becoming homeless within the territory served by CIUSSS West-Central Montreal.

Mission

Centring the user’s needs, the Connexion team offers psychosocial and transitional primary healthcare in the community to individuals experiencing homelessness or at risk of becoming homeless. Its goal is to connect these individuals to social services or community resources that can sustainably meet their needs or situation and help them off the street.

Connexion’s interdisciplinary team includes:

  • Homelessness pivot workers
  • Social workers
  • Clinical nurses
  • Psychoeducators
  • Human relations officers specializing in addiction
  • Peer supporters
  • Consulting physician

To receive support from the Connexion team, make your request with the community resources providing your meals or housing.

If you are experiencing housing precarity, at risk of eviction, or experiencing homelessness (or if someone close to you is), you can contact a psychosocial support worker 24/7 by calling Info-Social at 811, option 2.

You can also go to the psychosocial reception desk at one of the CLSCs within our CIUSSS network.
