What is the mission of our Adult mental health (AMH) teams?
The AMH teams offer services to clients with a mental health diagnosis or symptoms of a mental illness.
Short-term care (maximum 12–20 sessions with an occupational therapist, psychologist, or social worker) for a client presenting with: light/moderate symptoms of a mental health disorder.
The referred user must have a capacity for introspection that enables them to establish short-term mental health recovery goals.
Nursing follow-up: for severe and persistent disorders in stable condition.
Requests processed by Adult Mental Health (AMH) come from the adult mental health access portal (MASMA) and other internal services and departments.
Admission criteria
- Must be aged 17.5 years or older at time of request
- Must have a mental health diagnosis or persistent symptoms of a mental illness that interfere with functioning
- Must be mobile or able to use technology for virtual follow-up
Services are offered at four of the five CLSCs in our CIUSSS. Users who live in the territory covered by CLSC René-Cassin are referred to CLSC de Benny Farm.
The CLSC de Benny Farm adult mental health team is comprised of:
- Social workers, social service workers
- Psychologists
- Occupational therapists
- Nurses
The CLSC de Côte-des-Neiges adult mental health team is comprised of:
- Social workers
- Psychologists
- Occupational therapists
- Nurses
The CLSC de Parc-Extension adult mental health team is comprised of:
- Social workers
- Psychologists
- Occupational therapists
- Nurses
The CLSC Métro adult mental health team is comprised of:
- Social workers
- Psychologists
- Occupational therapists
- Nurses
